The position is at the Directorate of Social Services, which has a threefold mission: to provide support for patient care (reception, flow and transport of patients) and to integrate and ensure social follow-up for HUG patients and their families while supporting clinical activity in logistical aspects. It is divided into four entities, namely flow management, reception and telephone central, intra-site patient transportation, and social services. As a social assistant at HUG, you intervene with patients and relatives within the department of social services in a global and multidisciplinary approach. You are responsible for the social follow-up of hospitalized individuals, particularly in care units and LSM beds (beds for social placement). Your role consists of assessing the social needs of patients and their relatives, identifying social difficulties, and proposing appropriate solutions. You assist in managing requests and financial matters and support individuals in their processes for maintaining home living, transitions to another establishment (EMS, care structures). You work closely with the internal multidisciplinary team as well as with external partners. Your work is based on individual, family interviews and multidisciplinary conferences to establish a relationship of trust with the patient, understand their needs, and develop concrete and personalized solutions. You are also responsible for the rigorous transmission of the social needs and projects of patients to the healthcare team and partners, actively participating in professional monitoring regarding social matters and social insurance, and the prevention of social problems. You are involved in continuous training within the team. You hold a Bachelor’s degree in social work or an equivalent title, possess in-depth knowledge of the social network in the Geneva area and of social insurance, and have experience in social work in a hospital setting and/or in social security. You are a person with excellent abilities to constructively advocate for social expertise in a demanding and emotionally charged interdisciplinary environment, have a sense of priorities, and can work autonomously. You have excellent writing skills and mastery of tools related to social services or social insurance represents an additional asset.
