The Health Network la Côte (RSLC) is one of the 4 networks of care in Vaud according to the Cantonal Law on Health Networks (LRS). It groups together regional care providers (hospitals, EMS, CMS, doctors, etc.) and municipalities. The mission of the network is to improve, in close collaboration with its members, the care and the users in the X system. RSLC manages the Regional Office (BRIO) whose main missions are: assisting people with loss of autonomy upon discharge from the hospital, managing and processing requests for accommodation in EMS X, handling social-medic requests, and completing its social offer with its partners Hospitals and medical practices. The Health Network la Côte is looking for a social worker (assistant-e social-e) on a permanent contract.Your main activities:With the acute care hospitals of RSLC:• Ensure the processing of social and insurance requests to be handled during hospitalization in collaboration with caregivers (including liaison services).• If necessary, direct to community social services.With medical practices of RSLC:• At the request of the physician, ensure the treatment (initiation of procedures and/or referral to other professionals) of social and insurance requests to be handled for adults whose access to care is compromised by social and/or financial difficulties (housing, lack of insurance, lack of specific knowledge and/or support for example).• Be a resource person for social questions for hospital services and medical practices.Profile:- Degree in social work (Bachelor of Arts HES-SO, social services) or equivalent qualification.- CAS specialist in professional analysis in the field of social action, or addictions.- At least 2 years of experience in the health sector (hospital, extra-hospital, and/or EMS and/or medico-social institution), as well as in network work.- Very good knowledge of social insurance, the Vaud medico-social system, and procedures with the Justice of the Peace.- Good writing skills and mastery of usual tools.- Proven ability to work in a multidisciplinary team and network.- Proactivity in the area of expertise.- Good listening skills and a vehicle with a driving license.What we offer:- A pleasant working environment within a dynamic team.- Social conditions according to the CCT of the Vaud parapublic health sector.- Opportunities for continuing education.- Start date: as soon as possible.Complete application to be sent by March 20:For further information: Ms. X BRIO.
