The Medical-Social Centers have the mission to help people of all ages who are dependent or have health issues to remain in their living environment for as long as possible and to support family caregivers. ASPMAD, with its 9 CMS X 950, takes care of clients at home annually and serves 105 municipalities in the North. We are looking for a social worker for our CMS X Vallée de Joux. Start date: July 1st.Your mission:As a social worker, your role involves advanced mastery of specific skills:Needs analysis and social intervention: You identify and assess the specific needs of clients to direct interventions within an adapted social health framework, including personalized follow-up according to each client's needs.Support for family caregivers: You provide advice and support to relatives, while offering resources to improve the well-being of those involved.Interdisciplinary teamwork: You collaborate effectively within a multidisciplinary team and with an external network for a coordinated approach.Case management: You actively manage client files acting as a point of reference.Your profile:You hold a diploma as a social worker or an equivalent recognized title, ideally supplemented by two years of experience. You have a solid knowledge of social security and the socio-health system of the canton. A certificate in social security would be considered an asset. You demonstrate autonomy, versatility, and ease in communication. You have a keen interest in home care as well as social security and inclusive practices. You present a great capacity for listening and communication within a group and with clients.
