The Saphir Foundation, a multi-mission and multi-site foundation (psychogeriatrics, geriatrics, adult psychiatry), is active in the medico-social field in the canton of Vaud and in the reception and support of people weakened in their physical and mental integrity during their life process.
We are looking for our establishment EMS Maurice Bugnon in Yvonand, which accommodates 84 people and specialises in geriatrics:
Community Health and Care Assistant (M/F/D) on a permanent contract between 80% and 100% - EMS Maurice Bugnon in Yvonand
Whatever their role, the professionals of the Saphir Foundation share a common will, that of supporting people daily to enable them to concretely experience the institutional service strategy "like at home". That is to say, to preserve their habits as much as possible, to offer a response adapted to their wishes and, through genuine interprofessional collaboration, to ensure the well-being and happiness of the people.
As a community health and care assistant, you will be responsible for supervising and supporting people in daily life and will take part in their overall support by assessing their physical, mental, social and spiritual needs. You will participate in the development of support plans and provide delegated medical acts under the delegation of the qualified nurse and will prioritise interprofessional collaboration. Furthermore, you will participate in the supervision of new professionals, apprentices and interns in the support service.

* Federal Certificate of Capacity (CFC) as a community health and care assistant
* You have an interest in the medico-social field and particularly geriatrics

* You master common IT tools (Carefolio) and have a perfect command of the French language
