You will participate in the evolution of the home care and community health sector by responding with us to the needs of values: TRANSPARENCY – EQUITY – RESPECT - TRUST.DIABETES SPECIALIST NURSE - 80% - CDIMissionIn the medical framework and closely collaborating with the treating physicians, referent nurses, and the client's entourage, you will ensure a role as an expert in diabetes within ASANTE in specialized care. - Ensure clinical care for diabetic clients at home.- Develop individualized care plans.- Provide specialized and therapeutic care.- Support clients (individually or in groups) in the development of their autonomy and prevention.- Support teams in clinical analysis and management of complex cases.- Promote the development and updating of skills in diabetes care.- Facilitate training for interns and contribute to the dissemination of best practices.- Collaborate with other specialists (dietitians, clinicians, IRPC) and the clinical pool towards regional and cantonal objectives.- Represent ASANTE SANA within the cantonal diabetes group.Location:Across the entire sector covered by ASANTE SANA (Aigle, Rennaz, Montreux, and Vevey).Profile:- HES Nurse or equivalent title.- CAS in diabetes (or equivalent).- Experience in home care settings, with clinical analysis and reasoning skills.- Autonomy, rigor, and a sense of teamwork and knowledge
