We are looking for the CMS of Avenches, a:
mental health specialist nurse between 60% and 80%
Your missions:
Ensure clinical assessment, follow-up and adaptation of care for clients presenting mental health issues at home, in a person-centred approach
Coordinate and lead the care project in close collaboration with the client, their family, medical staff and network partners, in an interdisciplinary dynamic
Plan, organise and carry out interventions combining mental health care and general care, ensuring continuity, quality and safety of services
Promote autonomy, home maintenance and overall well-being of clients through personalised support adapted to the evolution of their situation
Your profile:
Hold a recognised HES nursing diploma by the Swiss Red Cross or an equivalent qualification
Preferably have specific training in mental health, equivalent to a CAS
Ideally have at least 2 years’ professional experience in general care and/or adult psychiatry for the elderly
Be able to work independently and interdisciplinarily
Have a reflective practice with analytical and synthesis skills
Organised and rigorous, you know how to manage priorities efficiently in a demanding environment
Comfortable in communication situations, able to establish trust and conduct constructive exchanges with diverse interlocutors
Hold a driving licence and own a vehicle (possibility to use a company vehicle)
Knowledge of German is an additional advantage
