In order to complete our team in Renens Sud, we are looking for, immediately or as agreed, a:Referent Nurse in Health Promotion (60%)As a Referent, you will ensure prevention and health promotion activities for clients with health concerns, in order to promote their long-term home care:- Ensure the monitoring and management of care- Ensure follow-up with the client, their relatives, the multidisciplinary team, and partners- Guarantee an updated client file- Conduct screening for vulnerability elements and risk factors among clients- Identify the client's and their relatives' resources and strengthen their ability to act- Implement actions to ensure home care according to the quality of life desired by the client- Strengthen the client’s connections with their formal and informal networks and assist them in their démarchesProfile and Skills:- Degree in nursing HES or a recognized nursing title in Switzerland- Minimum of 2 years of experience in the field of care- Good clinical analysis and synthesis skills- Ease in managing tasks and using tools for managing priorities and confirmed taking care of community care with public health actions- Ability to work in internal and external networks- Knowledge of the Vaud healthcare network is desired- Competence in the French language with good writing skillsWe offer:- Working conditions according to the Collective Labor Agreement (CCT) in the Vaud semi-public health sector- Professional insurance conditions that go beyond the minimums required by law- Five weeks of vacation per year and six weeks starting at age 50- Offers of rich and varied opportunities related to the position heldWorkplace: CMS X SudRate: 60%Start date: immediately or as agreedWant to join our team?
