Mission You will ensure the overall care of individuals presenting mental health disorders, both adults and minors, directly in their homes. Your role is central to maintaining home care, crisis prevention, and therapeutic support, in close collaboration with the medical-social network. Responsibilities: Assess the mental health needs of clients at home, including risks, familial and social environment. Ensure specialized nursing follow-up in mental health and establish and monitor individualized care plans adapted to each client. Support children, adolescents, and their loved ones in a holistic and therapeutic approach. Prevent crises, identify signs of distress, and intervene appropriately. Monitor the use of psychotropic treatments and track their effectiveness and side effects. Participate in interdisciplinary discussions (doctors, schools, institutions). Support caregivers and strengthen their skills in daily support. Participate in clinical meetings, handovers, and quality initiatives. Promote a person-centered approach that respects individuals' pace, autonomy, and family context. Your profile: Nursing diploma HES or equivalent title recognized by the Swiss Red Cross. Experience in the field is essential. Experience or strong interest in the ability to work independently. Good listening skills and the ability to manage complex and emotionally demanding situations. Team spirit, reliability, and adaptability. Valid driver's license is essential. Proficiency with electronic health records and care documentation.
